The Public Inquiry into the Mid-Staffordshire NHS Foundation Trust commenced today, Monday 08th November 2010, with Tom Kark QC as leading counsel to the inquiry and Chaired by S Robert Francis QC. 

The inquiry was announced following a report in 2009 condemning conditions at the hospital, "which are said to have caused hundreds of avoidable deaths"
